Two systems analyses for a state work-health-and-safety regulator.

Systems analyses our principal led at the regulator. Included because they show the method at sector scale: hundreds of participants and incident accounts, one systemic map per sector, recommendations at the level that can change the outcome.

Published work, 2022 and 2023 Both reports were published by the regulator and are free to read in full. The summaries here are in our words; the originals are theirs.

Who is in the system; how the contributing factors connect across its levels; and where prevention can act. The regulator’s reports are summarised here in our words, the originals are theirs.

Occupational violence and aggression in residential out-of-home care

Interviews, focus groups and workshops with more than a hundred participants across the sector. Five clusters of contributing factors: staff consistency and supervision, staff competency, client matching and mix, facility design, and incident reporting, investigation and culture.

Manual handling and psychological injuries in public hospitals

A systems survey of 173 hospital staff and seven stakeholder workshops. Three clusters for manual handling (staff availability and workload, learning and competence, environment and equipment) and three for mental injury (high job demand, occupational violence, bullying). Presented at the regulator’s health and safety month, 2023.

The regulator is WorkSafe Victoria, where our principal was Senior Specialist, Human Factors, in its Ergonomics and Human Factors Unit. Both reports are public: occupational violence and aggression in residential out-of-home care and manual handling and mental injury in hospitals.

The same method, at your scale.

Incident investigation and systems diagnostics use the same map, from one event to a decade of records.
